Far too many to say an exact worst, but, off the top of my head, from something I played somewhat recently, S.C.A.T nes. I also didn't play this one, but Dragonblade Wrath of Fire for the Wii.

I think it might be Code of Princess for the Nintendo 3DS. I saw it in Nintendo Power, thought the cover looked interesting. Thought the basic gameplay looked fun enough. I figured the plot would be braindead but I'd have a satisfactory beat-em-up because it seems like a hard genre to mess up.
I was wrong, it is a genre that can be messed up. I saw people talking about with less than stellar things but the game was on sale and I had a coupon so I got it for like $10. Not worth it. Very boring game.

Probably the Wizards Of Waverly Place game on the DS, I beat it in 3 hours and the game somehow STILL felt like a maze and I had absolutely no fun playing it lol
If you want a good DS Disney game for younger girls, play the Tinker Bell DS game, I unironically love it for no explainable reason.
Though I do remember there was also this Wii game called Puppy Luv I got many years back that was extremely boring, annoying, and never seemed to work. I didn't play it for very long, even then I was at the point where stuff like that wasn't very interesting to me lol

I may like the 2 shitty Crazy Frog Racer games but Crazy Frog Collectables Art School is god awful. I geniuenly think I can't find a physical game worse than this.

The only 'game' I have in mind right now is Renapet. It's actually a WIP & free, but there has been 0 updates since I played it 5 years ago, & the only feature that was developed on the game ( sodomy ) has been removed from the newer, only downloadable versions. Now the game is basically just a screen with buttons that make sound, but nothing else.
Apparently, the artist is working on too many projects & often puts them on hold. I'm really disappointed because I already played some of this artist's games & they were cool. Renapet would have been an amazing project, but it seems like the whole damn thing is just destined to be a Patreon trap.
I draw buff TF2 mercs & painful stuff. Beware.

Balan Wonderworld is technically the worst Square Enix game never seen my fuckin life, I accidentally play Balan Wonderworld i try to complete but T Pose, glitches, unpatch version, and eventually Yuji Naka leave Square Enix and send into the prison
I guess objectively, Sonic Forces? I'm not a huge Sonic fan, so I can barely tell the difference between a good and bad sonic game, but Forces had some weird level design and a plot I most certainly didn't care about. I only got maybe 5-6 hours out of the game. The character creation was super fun though!
