Like I suggested earlier, if medals aren't unlocking using Ruffle, try turning off Ruffle in your account settings so the game launches with NG Player. I was able to unlock a medal in this game that way.

The last secret medal seems to be broken, as I beat level 15 (the last level) in two different playthroughs, even after refreshing the page, and the medal didn't unlock at all.

I've made some research and found out a thread (started by you yourself), and one of the replies said that you have to play the game at the beginning of any month because of the Nutcasenightmare API failing later in the month. Just saying because I also found out about it just now. Getting medals in this site really is a harder task than it should be.
none of the medals in Witch Hunt seem to work, specifically the First Blood medal for killing the first enemy.

Actually I was wrong, it's got nothing to do with playing at the start of the month. It seems the problem is about your browser blocking insecure content, so you have to disable the protection. Here's how to do it in Firefox:

Oh my god it worked! Thanks a million, made my day finally resolving this.
Apparently coding is so old that it works differently. I'm more amazed this is the oddest trick to make it function. I may keep this in mind for the future NG games to attempt medals.

Medals in this one seem to be broken. Have got up to rank S+ twice and still not unlocking. Hasn't been an unlock for the rank-based medals in 2 weeks so either nobody's playing or it's broken for everyone. As you can see on my profile I managed to unlock some of the lower-tier medals a year or two ago, but it doesn't seem to work for me anymore.

He hadn't made any intentional updates recently but was wondering if he had some changes that hadn't been published until more recently, not sure if a fix will happen before the big update they've been trying to wrap up.

i also tried on an old account on a virtual machine
the virtual machine (linux) only had firefox downloaded, and i didn't get the medal on there either, on a friday night
just tested again now with my current account, and i didn't get the game starting medal
according to what tom fulp said, i think it's not just an issue on my end, it seems like it's been occuring to other people as well for a couple weeks now, and perhaps it's some kinda recently published minor update/changes that screwed with the medal system? i'm not sure

Checked the medals by tapping the walnut 100 times - neither "100 Points!" nor "100 Taps!" were awarded.

Seems something odd might be going on with The Interview Codex's score system - starting the flash once after deleting my scores gave me a score of 7, which went up to 14 after refreshing.
UPDATE: Refreshed again and saw my "Who visits the most?" score go up to 21 - seems its definitely going up in increments of seven, for whatever reason.
UPDATE 2: Checked on Brave - seems only Firefox is having this issue.

Went back, deleted my scores again and tried repeatedly opening The Interview Codex in NG Player - didn't earn "Knowledge Creeper" despite eleven visits.

Found out a couple of Rob1221's games are suffering from broken medals.
First's Polarity Switch, which didn't award me "12 Levels" despite having finished every level up to Level 14:
Second is Dark Light Swap, which didn't award a medal after I'd finished level 4:
I did earn a couple medals on Polarity Switch back in early 2023, which narrows the search for any breaking updates somewhat.
There probably are more games of Rob's which suffer medals not being awarded, but these are just the two I noticed.

That's odd, I had this medal unlocked awhile ago.
There's a chance Firefox got an update and it did something but uncertain. You could attempt to get it on other browsers if doable.
