You'd have a hard time without finishing most of the other challenges in the game.
Hard mode story, secret ending (you can do it on normal if you like, but not on easy), and subsequently hard mode challenge 3. These will give you the items that you could really do with up against this boss.
Other than that, you could save yourself some health by making sure that your strong hand is always on the 'attack' key. Either swap the control scheme around on the main menu, or cross your arms over like I did.
Run around hitting boxes. Not only does it fill your carnage meter up, but they also sometimes hits him.
Avoid him at all costs. He's Death. If you have any lasers handy (which you might do if you follow the advice in my first paragraph), use 'em. Otherwise, throwing your weapon and the boxes at him is actually a great strategy (and increases carnage further as he attempts to knock them away)!

Firstly, choose a good weapon. I went with Dadscaliber II. Also, play on noraml mode
When you get him into 'boss mode', you should be on fire, so run with it. Your goal is to get in cheap shots, stay healthy, and run around increasing your carnage percentage. The guitar is good at that, but you may prefer to use it to do damage to him with it, by making one of the three rockers rock out.
Remember that pressing down makes you fall down quicker, allowing you to dodge his sidewards attacks. Try to stay high, but not at the same height as him (since his short warp attack is very quick and very painful). The item obtained from completing hard mode Challenge 3 I think will help. If you're running low on health, carry on running away, dodging and damaging things.
You may find some use in the charged attack for Dadscalliber II, since it does decent damage and may be good for a quick escape (though I can't be sure).
