By my guess, its probably a combination of the game being extremely popular online and the Song ID system making players quite aware of NG.
It does mean we have to deal with a glut of GD gameplay footage, but that's a price I'd willingly pay to keep NG from falling into obscurity and to earn some easy whistle points.

it's a duplicate upload/cut-down version of the second upload
there is no rule against doing this but the second item gets taken down as it is un-needed clutter in the portal
imagine if everyone did that
the original intention was to create an A-rated piece, so the artist should stay the course and create an A-rated piece, not try to create 2 different versions of the same animation, in the end, it is handled as general spam

Under the Movie Guidelines, this would be considered a variant. NSFW or not, the Guidelines says they should be opted out.
They are allowed on the Portal, but they shouldn't get in the way of brand new projects, as technically you're not adding that much new stuff into it (or in this case, you're taking something out).
It also did not say the original project needs to have won an award or be eligible for an award.
"If you are uploading something that could be considered a variant or re-upload of an existing project, please check the "Opt out of any awards" box at the bottom of your project details form. Variants include style variations of something you previously published, dubs in different languages or a re-upload of something you previously unpublished. If you are merely updating an existing entry, we ask that you update the existing file rather than create a new entry."
I also think the removal was done by a moderator, since I flagged it as a variant.
If the author removed it themselves, it will display this.

Tom and the editorial staff have let P-Bots stand of variants and have featured variants on the front page, so clearly there's ambiguity here.

The author is primarily an NSFW author, and both submission are still intended to be sexual in nature (note the tags).
Whether the loop version came first vs the dick version, or if only the loop version was uploaded, it wouldn't really matter. Combined with their short length, it'd be best to be safe then sorry.
