Tried screwing around in JPEXS to work it out, then gave up and searched through the reviews. Luckily, Ximolia worked it out before me, then TFZ9 copy-pasted it, so I'll do the same:

To get the secret medal, you need to go to the picture with the artist's name, Blounty. Click in the center of the left most eye.

Then click on the center of the screw in the ice cream.

Then click in the center of the screworm on the right side of the bump.

After that click slightly below the tonsil.

Level 44 was near-impossible. Level 49 was geniunely fucking impossible. I tried using a fucking walkthrough for 49, but the game's physics pushed me off and killed me if I did manage to land where the walkthrough showed me I needed to land.

On the bright side, I just crossed 5k Medals, and I've just hit Sgt. Major!

I've officially been a Supporter for 5 years! I also passed 26,000 EXP points, and am nearing 19,000 saves

Ran through a fair number of MrNannings games today, and got a lot of medals as a result.

Level 23 expects you to finish a far-too-long level with twenty blocks that are actually useful and fourty bullshit arrow blocks which are virtually useless as platforms. Fuck that.

The delay between enemy spawns extends with every wave, to the point of causing enemies to virtually stop appearing on Wave 10. This makes maps practically unfinishable.

Nice work on that! Some of those are pretty hard to find.
