Those two demons in the Alchemy Laboratory that double-team you in Castlevania: Symphony of the Night
Darkside and Guard Armor in KH1 when you're doing a Proud Mode +Magic -Defense run. These bosses have a very creative set of special attacks, some of which can't be deflected. Meanwhile you're stuck with a dinky 3-hit combo with horrible start lag (and ironically no magic despite picking +Magic) and you can't even dodge roll until AFTER you beat Guard Armor. Your HP already melts from the slightest enemy tap on Proud, but picking -Defense guarantees that those first two bosses will push you to consider restarting on a lower difficulty.
For what it's worth the difficulty takes a nosedive afterwards so it's better to just thug it out.

I mean it’s not really a first boss but fuck it, the OG Persona 4 on PS2 had this bullshit 3rd boss fight after like 4-5 hours of gameplay I think.The boss didn’t have any weakness to attack at all, and I was playing on Expert, the difficulty was permeant unlike Persona 4 Golden. I was also under leveled as shit, took me 10 hours to beat that boss, fuck whoever decided not to give the boss a weakness
The Ant Lion boss in Sonic 2 on the Sega Game Gear. You get no rings in the level whatsoever, you're on a slope the entire time, making your momentum slide into the ant lion almost constantly if you're on the bottom half of the slope, and cannon balls bounce down with different, random patterns, making how to react difficult. This boss is abysmal and I have never got past it a single time
the freaking cleric beast in bloodborne!?? why?? that was actually so difficult. (Then again, it was the first time i played bloodborne[did play other souls games tho...]) He is so agressive and does a lot of damage. trying to take him down was difficult as the weapon you had was did not do much damage. had to use molotov cocktail

well this may not count yes and well you will find it funny yes but for me it was Joe Cobb yes in Fnv because then when I first met him I didn’t known what he was doing and I shor him several times and he kept killing me yes and I thought it was hard
I think the bird monster in Super Ghosts & Goblins is hard, but I managed.
For a strange, hard first boss:
The mutated Comms Officer in Resident Evil Revelations.
That bloated idiot is the definition of “bullet sponge”.
Phrike in Returnal
Returnal is a brutally hard game when you first start playing.
If you die at any point during a run, you have to replay the entire game.
...and with the exception of a few insignificant things, nothing really carries over after death in the first section of the game.
Beating the boss grants a key to access the next area, which is a permanent item (you keep it even if you die).
He is essentially guards the first "checkpoint," and is easily the most difficult hurdle to jump over in the game (until you clear him for the first time). You can prepare for literal hours... but if rng doesn't favor you on your run, you are in for a very hard time trying to tackle this guy.

For a first boss Peppermen is pretty damn hard especially when he heals all the way up and becames two times faster during the second phase. Can't forget about the ending too where he is tiny and annoying to try to hit.

I mean Pepperman is just a little test of getting good, and it actually takes attention and tries to get it, although his attacks are actually easy and once you understand what's going on you only need to taunt or move away.
And the last attack? You only need to make him jump then attack, ez

That's why I said "for a first boss". Pizza Tower is a game that takes a while to get a handle of the controls and you can fight Pepperman after beating two levels.

Not a "first boss" per say, but that initial bullet hell section in Nier Automata on hard or very hard is something that made me cry in the bathroom.

For me, Cerberus in the Coliseum was that point where I almost rage quit. No Dodge Roll, limited magic, and it was a pain in every phase.
But honestly, I love this game for moments like that, when even simple bosses turn into total nightmares because of your build.

The first boss in Miscrits (a game who got removed but now it's coming back now) like this boss is literally a hard rock to budge like, I gotta grind for a long time

do sequence breaking 1st bosses and newbie traps count? because of how progression works in Terraria, any of the pre-hardmode bosses can be your 1st boss, and picking (or accidentally stumbling on) the "wrong" 1st boss will result in you having a bad time.
the newbie traps:
king slime is one of the easiest bosses in the game (a giant slime that teleports and produces more slime enemies as it loses health), but can be a real pain to blind players who get caught off-guard with no way to escape it or accessories/weapons to help against it (did you run out of shuriken mid-fight? sucks to be you).
the deerclops from the don't starve crossover is very easy once you figure it out, but until then, it can be one of the hardest pre-hardmode bosses in the game, since it can catch you off-guard just like king slime if you don't know how to avoid the spawn area when the conditions favor it. even when you got it figured out, fighting it without proper mobility can be a pain, as it can freeze you if you don't dodge in time.
the boss in the evil biome is not hard under normal conditions (the corruption spawns a giant worm that splits itself in multiples if you kill the wrong segment and the crimson spawns a huge flier surrounded by loads and loads of flying minions), but it's easy to awaken by accident and if you don't know what you're doing... good luck, you'll need it.
the sequence breakers:
skeletron is a boss i normally challenge after beating at least 2 bosses (which grant access to better weapons and armor) and an invasion which unlocks accessory combination for better mobility, but there's nothing stopping you from picking it as your 1st boss, which can be difficult if you didn't get the recommended health upgrades, given the 3 flying hitboxes you'll have to constantly dodge while trying to kill the head before dawn breaks and it enters insta-kill mode. even worse in expert mode where it gains a ranged attack.
the wall of flesh, is supposed to be the final pre-hardmode boss for a reason. deciding to make it your 1st boss is suicide, not just because the environment and fight itself (have fun running away from it over large pools of lava and irregular terrain, or worse: buildings), but also because you're unlikely to survive the things that spawn upon defeating it, should you succeed. and did i mention you can end up accidentally summoning it if you have minions around to protect you? and that if you try to escape the fight you die?
