Kakihara commits suicide after realizing Ichi is too weak emotionally to kill him, the scene of Ichi kicking him in the head and knocking him off the building is a hallucination he has after driving the needles into his brain. This explains why when Jijii discovers his corpse there is no gash on his forehead.
That's only one interpretation, seeing as Kakihara is also shown sitting in the blue room later in the film. But I can definitely tell you that the man hanging from the tree is Jijii and the kid walking away from the body is supposed to be Ichi.
