It happens. They become harder to find if it's not caught soon after it's uploaded. It's like a snowball effect, the longer it's gone unnoticed the less likely it is to be caught.

Honestly, a moderator would have to check the song and the link given just to be sure that it's not a mistake by the user who posted. In most cases, that is.
In the case of a song that's been uploaded a thousand times, they can hear the first few seconds of the submission and be like, "yup, it's stolen."
Hence why they would "sleep" with most songs.

The real issue with the audio in question right now is that the uploader did not do the remixing, but rather ripped it from YouTube.
