Only flashes WITHOUT icons will be updated to your 140x90 image, even if it is 46x46. A flash that already has a 46x46 icon will not change to the one that you have uploaded regardless of if it has a 140x90 image or not.

If you put 140x90 on the movies "Child One" and "Invasion of Random Things" only Child One would update even though they both don't have 140x90 icons made for them yet, only because Child One doesn't currently have a 46x46 icon at present.
You was probably mistaken and one of their icons didn't have an icon where you said there was.
