Since it's not a secret medal, there is a secret tunnel in the middle of the staircase. Walk the guy down the stairs and push left on the middle stair to reveal it.

And what about the ghost walls?! Do they disappear once you finished hard mode?

It'd be great if there were a way to search within the medal list. As it is, I see ways to sort what you see, but not to search JUST within that category.

Medal_11 - Go to the top of the tower and use cross to find the secret in top left corner.
Medal/20 - I got this by hitting Medusa head with my head in first level. Just jumped, died (did this on REAL MODE) and got it.
