Okay now I am a bit confused. There are several people who unlocked the level 1 easy /2 normal /3 hard /4 insane medals. I tried everything on every difficulty, all on insane without a continue, too. But nothing unlocks. How can you get them?

I just played through the game on insane difficulty to unlock them all, with plenty of continues unfortunately. The medals for score seem much more difficult to attain.

The weird part is that it worked when I played through six levels before I stopped for a while because I had something to do. Once I got back just now, all of a sudden the medals stopped working for me, what gives?

You need to push both the blocks down, then hit the top block left and the bottom one to the right. Hit both the blocks down, then push the right block left so it hits the other, then push it up.
