You can find it in level 13 after playing the game for the 2nd time (I think). It will show up when you collect all coins and kill all mobs in that stage.

Not sure, but i think you have to get a million points for it. The instructions on how to get it were in the endgame scene, which I blanked out on.
I hope you've all not only played medal games, but also watched a few flash animations last year.

You are right... I have to get 1 million points in the endgame stage, which is impossible?
Unless there is a trick to it or 1 million totally (repeat the stage).
Other than that, it's worth it.

Not really. I've just been going through the study mode of it, and hoping I can figure out the faces with the tests hints.

I've only one that you don't have (I'm missing the 3rd one), and with that one, the only hint I'll give is kinda cryptic... but you don't have to listen to the judge. :)
