After a couple of days, I finally perfected Tired Old Man in SCGMD4. Nothing that musical practice can't defeat, and it's vastly easier than playing a piano sonata or concerto at any rate. Shame that a video of this sort doesn't actually serve as a walkthrough =(

I anticipate that the last two songs will not take long compared to Tired Old Man, though unfortunately they're more endurance type songs than actual challenges.

It's been there for a while now, but only recently did its medals start to work. He even added a dozen more medals to it. I've been steadily working on it for a couple of nights, as it is ultimately a grinding game.
Some pointers:
-- Weapon upgrades ups the maximum level you can achieve for the weapon while playing. This goes up to 21.
-- Weapon potential is limited by the maximum level of your weapon and your secondary/third weapon slots as seen in shop. Your primary weapon does not have a level limitation, although your weapon maximum level can still inhibit the equipped weapon's potential.
-- When you unlock a weapon, the starting max level is 12.
-- Expensive weapons can destroy enemy bullets.
-- You can equip multiple copies of a weapon. For example, if you just unlocked the Beamer weapon, you can equip all three of your weapon slots with the Beamer.
-- Increasing the difficulty level causes enemies to have more health, and more enemies will appear.
-- A full run on normal mode earns you roughly 48k money. A full run on hard mode earns you roughly 79k. I'd hazard a guess that score is directly related to your money earnings, although I'm unsure how to get a really high bonus. Hoard lives and bombs maybe?
-- So far, every 10 experience levels increases your starting primary weapon level. When you start a new game, your primary weapon level is 1. If you get to experience level 10, when you start another game, your primary weapon level will now be 2 instead. It is still possible to lose those weapon levels from dying though.

Heh, you're still missing a LOT of games. It sucks to not know what games have working medals and what games have broken ones (the reason the medals were invisible before the redesign). I've been saying this since the redesign, because at least 50 games had visible medals and almost all of them were broken. As usual people didn't reply so I stopped caring about it as well. :)
If you go through Banglaboys games you'll find at least 10 games with working medals. After a while, I even started to like them. =)

Hehe, I'm sure I miss a bunch as well. I know for a fact that some of the games you listed have broken medals, but there are also some games I haven't played yet. I played Book of Mages: Dark Times for a few hours as it is a fun game. It has 33 visible medals but none of them work. Rather than listing games that might have working medals, it seems smarter to make some sort of database of games with non-working medal games that are currently visible in the Games with Medals list. Like I said before, that list is going to be at least 50 games long, but probably closer to 75, or even worse... =/

That's what I do. I don't want to waste people's time playing through a game for medals, only to not have them work.

Okay, see, I opt to post medal games that have point value medals in them, regardless of whether they work or not. The fact that I haven't done so in the past (iirc) is more due to me being lazy. When I play a game and find that medals don't work, it mostly boils down to either the NG servers derping or the author didn't do it right.
If the NG servers are indeed derping at the time that I play the game, then posting here basically opens it up to people to verify whether or not the medals work for them or not. I'm just one guy, and while I could play the game again at another time, why not gather a consensus on whether the game works as intended? After all, it is possible that the NG servers derped for the next guy and the next, but given that we have players from all different time zones and whatnot, I think the coverage is pretty good to get past this particular problem.
If all the medals are genuinely broken, then I hope to drive momentum to maintain the medal system. Either PM the author and ask him to check out the issue, or get the broken medals removed. I don't know what the NG staff's policy is on this stuff, if such a thing even exists, but at the very least PG seems interested in acknowledging the fact that there are broken medals, and that they're undesirable. For example, Knuckstrike found the game PRO-R by tedgaming. Before he made the post, the medals were broken and didn't work. However, it was literally only a day ago that the author responded to my PM and fixed the game, even adding many more medals to the game. Whether Knuckstrike would have posted the game when he found it earlier is something for him to say. But I don't want flash devs to attempt ways to gain exposure to their works via medals, only to never find out that they really don't work for the audience.
Broken medals are pretty rampant as it is, and the problem is only going to get worse. The way things are going currently, your suggestion of keeping a database of known broken medals would definitely be the most efficient way to keep all the medal wh0res informed. But wouldn't it also be nice to keep the clutter down by removing broken medals, or even better, broken games? I'm sure whoever is working on the medal system still has their hands full at the moment, but having broken medal games, 0 pt medal games, or no medal games clogging up the gameswithmedals list is pretty stupid, and I want to do something about it. Giving exposure to the magnitude of the issue is a start.

A couple of notes on The Love Letter.
1.) The Germaphobe medal is easily the hardest medal in that game. If you ever get trapped in a spot, you're done. Those people will follow you into the end of time, unless they lose you. And, it is seriously possibly for EVERYONE to crowd around you. Also, people seem to love running into you for some weird reason. This is the one medal I have yet to obtain.
2.) The <3 medal isn't too difficult to get, once you know the trick. You can hide in the cubby on the lower-left hand corner, most people won't come far enough to see you. If they do see you, you can lead them around the corner, and then head right back; they should lose you before they get into the cubby. However, to pull this off without getting caught by <=3 people seeing you at once requires a good starting locker location, as 5 people can come out of one room at the beginning and ruin your run immediately.
3.) I'm not sure if it's just me, but the Bookworm and Speed Reader medal seem to rely on luck. There are some small steps you can take (like picking a good spot, and leading people away from it), but most of it seems to come down to how lucky you are. You should get both of these in the same run.
4.) The kitty is randomly hidden in one of the lockers (The location changes each time).

I'm inclined to think he's just ungodly good (and probably has more time). Getting that many medal points is mostly about dedication and persistance imo. I know it took me a while to get to the spot I was at.

I think i found the same glitch, but it took me a while, and it was purely by accident. Altho, since you have all the medals, perhaps you can PM me the secret one (or if you don't know it, someone else who does.

I too would like to request if possible some info on the secret medal for this game. I know the medal itself says "PL", and seeing how the other medals have the letters the medal stands for (for example, the one medal "TS" stands for Time Stopper), I figure it has something to do with those letters. The only thing I can think of with a P or L is the Play button, and the Leaderboard. But damned if my tiny brain can figure it out.
Also, one last request if I may... does anybody have a walkthrough for the game "Spot the Differences"? I can get all the way up to level 9 (the one with the car picture), but cannot for the life of me figure out the last difference. I haven't even made it to level 10 yet because of that.
