Since medal incorporation is now public anyone can include medals into their games. Sometimes the game's medals don't get approved by admins and they stay at the zero point values. Hopefully there will be a better way of sorting this out in the future.

No, all of them work. To make sure they send, save the game BEFORE you are about to get the medal, then get it. If it doesn't send, you can refresh and try again. I got all of them. (GRINDFEST IS very time-consuming)

Also medals are sending out of order for me. As well, you seem to get the medal for clicking the okay button, regardless of if the answer is right or not.
And lastly, there is a medal of a girl with blue hair. But when you hover over it on your profile, it doesn't have any description / points. Not sure if thats a problem you want to sort out or not. Probably going to wait for a walkthrough for this game.
