My guess is that I get them all 3 at once when I start a new game. ;)

Only on very rare occasions has that happened here. Not only because programmers are generally lazy and can't be bothered to add a few lines of code that check for completion when you access the achievements page, but it makes it too easy for people to cheat. Although with my luck lately, odds are i'll be wrong haha.

You seem to think there is a trigger that stops the wheel from spinning, but it's just the other way around, you need to jump over the trigger so it doesn't start spinning. :)

Getting the invincible medal wasn't that hard, but it's very easy to make a mistake.

Just go and attack and retreat predicting his attacks to build up rage. when full activate it attack it twice, then jump over it and repeat. If you get hit then be more careful.
