Just goto Stitchimage.com or the link I posted on the first page, choose a color and a size, add the had to your cart, when the cart screen pops up, hit the checkout button. You'll then be taken to a screen where you can choose to pay by credit card or a paypal acct. On the checkout screen there is a text box where you can specify what you want on the hat.
The site will be getting a flash upgrade soon and will be a lot easier to navigate, especially when it comes to ordering a specific hat, like a NG hat or an Angry Face hat, etc etc.

That measurement is the diameter, get a tape rule and put it around your melon, then whatever size you get use it to judge which size is best for you. Remember, the hats have spandex in the rim so they stretch nicely, so if you're inbetween then get the smaller size.
